We are seeking a skilled and compassionate Registered Nurse to join our team as a Disability and Rehabilitation Nurse. In this role, you will provide specialized nursing care to individuals with disabilities and support their rehabilitation journey. You will work closely with patients, their families, and interdisciplinary teams to promote optimal health and well-being for individuals with disabilities, and help them achieve their highest level of functioning and independence.
Responsibilities:
*Provide comprehensive nursing care to individuals with disabilities, including assessment, planning, implementation, and evaluation of care plans.
*Collaborate with patients, families, and interdisciplinary teams to develop and implement individualized care plans that address physical, emotional, and social needs of patients with disabilities.
*Administer medications, perform procedures, and provide treatments as ordered by physicians or other healthcare providers, and monitor patient responses to interventions.
*Monitor and manage complex health conditions related to disabilities, including but not limited to mobility challenges, wound care, pain management, and behavioral health management.
*Provide education and support to patients, families, and caregivers regarding management of disabilities, including promoting self-care, independence, and optimal health outcomes.
*Assist patients with activities of daily living (ADLs), such as bathing, dressing, feeding, and mobility, as needed.
*Collaborate with other healthcare providers, such as physicians, therapists, and social workers, to ensure coordinated and holistic care for patients with disabilities.
*Document assessments, care plans, interventions, and outcomes accurately and timely in electronic health records (EHR) or other relevant medical software.
*Participate in ongoing education and professional development to maintain current knowledge and skills in disability and rehabilitation nursing.
*Adhere to infection control protocols, safety standards, and hospital policies and procedures.
Requirements:
*Current registration as a Registered Nurse with Sydney’s nursing regulatory body.
*Minimum of 2 years of experience in nursing, with a focus on disability and rehabilitation nursing.
*Knowledge and understanding of disabilities, including common health conditions, medications, and management strategies.
*Excellent communication and interpersonal skills, with the ability to effectively communicate with patients, families, and interdisciplinary teams.
*Ability to provide patient-centered care, promoting independence and self-care for individuals with disabilities.
*Strong critical thinking and problem-solving skills.
*Proficiency in using electronic health records (EHR) and other relevant medical software.
*Current Basic Life Support (BLS) certification.
